Alive certificate

Required documents for Attestation of 'Alive Certificate' for Bangladesh citizen or Bangladesh origin Canadians:

Submit duly fill in prescribed form. Alive_certificate_form.pdf
Submit one copy of the recent passport-size photograph.
Copy of valid Bangladesh passport.
Proof of valid status in Canada.
Proof of residential address in Canada
If the applicant desires to receive the service by mail, then a pre-paid express post envelope having a detailed address of the recipient must be submitted to the High Commission. In that case, a recent doctor’s certificate must be submitted with the application.

Fees:

URGENT	REGULAR
Urgent delivery fees within three business days from the date of the receipt of the application by the High Commission	Regular delivery fees within 21 business days from the date of the receipt of the application by the High Commission
C$ 60	C$ 40

The High Commission does not accept any cash or personal Cheque.
Bangladesh High Commission accepts payment of consular fees by debit or credit cards at the consular counter of Bangladesh High Commission, Ottawa.
Fees for consular services are also accepted by bank draft/money order/pay order/postal order made in-favour of "Bangladesh High Commission, Ottawa, Canada"
Fees are non-refundable and rate is subject to change by authorities.
